Telehealth systems since the introduction until now have always encountered problems to their adoption. The multiplicity and conflict in users' view, needs and concerns regarding the change in the way health services are provided makes the design of telehealth systems face a complex situation, which is defined as a soft problem. In this research, a hybrid approach, including soft systems methodology to identify problems, define requirements and actions along with the NASSS framework as a theoretical lens for guiding the participants' views has been introduced. This approach has been used in the development of patient telemonitoring systems with a focus on system adoption. In this study, actions including conducting interviews, drawing a rich picture, analyzing the content of users' views and identifying problems in the finding phase, and root definition, CATWOE analysis, and presenting conceptual models in the modeling phase were carried out. Next, in the phases of discussion and definition, based on two conceptual models from the physicians and patients' views, desirable and possible actions were defined. The result of this research is to present the expected features of the system, implementation processes, rules and guidelines as well as the policies required for the design and development of the patient telemonitoring system.
